Fatty Liver: Fatty liver is common, usually has no symptoms, and improvement is linked with weight loss and exercise.

~1 in 3
About 1 in 3 adults worldwide has fatty liver. Twenty years ago it was about 1 in 4 (Younossi 2023).
7 to 10%
Losing 7 to 10 percent of your body weight is linked with less liver inflammation and scarring. Losing 3 to 5 percent is linked with less liver fat (NIDDK).
55 to 68%
Between 55 and 68 percent of people with type 2 diabetes also have fatty liver (Younossi 2019).
Start with the signs
The signs of fatty liver.
Fatty liver means too much fat is stored inside your liver cells. The common form is linked with weight, insulin resistance and blood sugar rather than alcohol. It used to be called NAFLD, and since 2023 it is called MASLD, metabolic dysfunction-associated steatotic liver disease. Most people have no symptoms, which is why it usually shows up on a blood test or a scan. Tap the signs you have to see why they happen.
Nothing you tap is stored. This is a mirror for you, not a form for us.
What the evidence says
Which fatty liver claims hold up.
Fatty liver is not just an alcohol problem. It is not harmless. No cleanse clears it. Weight loss is what the research supports most.
These summaries reflect the cited research and will change if the research changes.
How it happens
How fat builds up in the liver.
Extra energy and insulin resistance push fat into the liver. Fat stored in liver cells is called steatosis. In some people it drives inflammation and, over years, scarring. Click through the four steps.

Why the early stage can improve
Fat in liver cells is stored, not fixed. When energy intake drops below what the body uses, the liver burns and exports stored fat, and steatosis can fall over the following months. Exercise adds to this even before the scale moves. Inflammation tends to settle once the fat load drops. Scarring is slower and improves in fewer people, which is why finding fatty liver early matters.
The four stages
The stages of fatty liver.
Fatty liver has four stages. Most people stay at the first and never move on. The stages are about how much inflammation and scarring there is, not how much fat. The earlier the stage, the more weight loss and exercise can change it. Tap a stage to see what is happening.

Medical care and lifestyle
How fatty liver is found and managed, and what fitness and lifestyle change.
Fatty liver is found with blood tests and scans, which also rule out other causes and check for scarring. Weight loss and exercise are the mainstay of care, so most of the work is daily habits. Both sides are needed.
What tests and medical care do
- Finds it. ALT and AST are liver enzymes on a routine blood test, and a raised level is often the first clue. An ultrasound shows fat in the liver.
- Checks for scarring. FIB-4 is a score worked out from age, ALT, AST and platelet count that estimates how likely scarring is. A FibroScan is a special ultrasound that measures liver stiffness, and stiffness rises with scarring.
- Rules out other causes and manages the rest. Your doctor checks for alcohol, viral hepatitis and medicines, and manages blood sugar, blood fats and blood pressure, because heart disease is the main risk with fatty liver.
- In March 2024 the first medication for the inflamed form with moderate to advanced scarring, resmetirom, was approved in the US. It is prescribed by a specialist and is used with diet and exercise, not instead of them.
What fitness and lifestyle add
- Weight loss is the mainstay of care in the AASLD guidance. 3 to 5% is linked with less liver fat, and 7 to 10% or more is linked with less inflammation and scarring.
- Exercise lowers liver fat on its own, even in the weeks the scale does not move. Both cardio and strength work count.
- Cutting sugar-sweetened drinks removes the fructose the liver turns into fat. A Mediterranean-style pattern is linked with less liver fat and better heart health.
- Alcohol kept to a minimum, because even moderate drinking is linked with more scarring when fatty liver is already there.
Your side of the work
The habits that bring liver fat down.
Weight loss and exercise do the most. Sugar-sweetened drinks and alcohol do the most harm. Tap a habit to see what to do.

What we track
How to tell if your liver fat is coming down.
Waist, weight change, weekly minutes and sugary drinks are yours to track. Liver enzymes, triglycerides, HbA1c, FIB-4 and FibroScan come from blood tests and scans. Tap a marker to see what it means.
Waist Belly fat
- What it is
- A tape measure around your waist at the navel. Belly fat is the fat that sends fatty acids to the liver. 35 inches or more is linked with higher metabolic risk in women.
- Why it matters here
- It tracks the fat most tied to liver fat, and it moves before the scale does.
- How we move it
- Measure weekly under the same conditions.
